Position Overview
As a Community Health Worker, you will collaborate with a dynamic team of Nurses and LPNs to support clients' independence and their ability to remain in their homes. Community Health Workers deliver home support services, assisting with personal care and daily living activities while promoting maximum independence. They play a crucial role in the home health care plan for each client.

Key Responsibilities
  • Assist with daily personal care activities, including bathing, dressing, grooming, and oral hygiene
  • Perform household tasks to maintain a safe and healthy living environment
  • Prepare meals according to designated dietary plans
  • Provide assistance with mobility and transfers
  • Observe and report changes in client behavior and condition
  • Execute delegated tasks in accordance with training and established procedures
This is a Relief Part-Time (0.90 fte) opportunity until the return of the incumbent. The shift types for this position include rotating days and evenings, with working hours ranging from 07:00 to 14:30, 07:00 to 15:30, and 15:00 to 23:00 hours.
