Job Summary

We are seeking a highly skilled and compassionate Access Navigator to join our team at Nova Scotia Health Authority. As an Access Navigator, you will play a critical role in providing exceptional patient care and ensuring seamless navigation through our healthcare system.

Key Responsibilities
  • Act as the first point of contact for families contacting our Provincial Preschool Autism Service
  • Explore child and family needs upon entry into our service and review their goals/priorities as needs change
  • Assist in matching child/family needs with available services
  • Develop and maintain professional relationships with families throughout their preschool years
  • Support access to school-age diagnostic services as needed
  • Participate in planning, development, and ongoing evaluation/quality improvement
Requirements
  • Bachelor of Social Work required (other healthcare professional degrees with experience in facilitating intakes may be considered)
  • Registration with the Nova Scotia College of Social Workers (NSCSW) or applicable College is required
  • Minimum of two years related clinical experience, required
  • Three to five years' experience in intake and/or client navigation, preferred
  • Experience in pediatric settings, and specifically autism, preferred
  • Knowledge of IWK, Nova Scotia Health, Autism Nova Scotia, Hearing and Speech Nova Scotia, and community resources and skills in navigating systems preferred
