Job Summary

We are seeking a highly skilled and compassionate Access Navigator to join our team at Nova Scotia Health and IWK Health. As an Access Navigator, you will be responsible for providing exceptional patient care and navigation services to families and children with autism spectrum disorder (ASD) in our province.

Key Responsibilities
  • Provide initial contact and assessment for families and children with ASD, ensuring a comprehensive understanding of their needs and goals.
  • Collaborate with multidisciplinary teams, including clinicians, administrators, and community partners, to develop and implement individualized care plans.
  • Facilitate access to diagnostic assessments, interventions, and support services, ensuring timely and efficient navigation through the system.
  • Develop and maintain professional relationships with families, clinicians, and community partners, promoting a culture of respect, empathy, and understanding.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices in patient care and navigation.
Requirements
  • Bachelor of Social Work or equivalent degree in a related field.
  • Registration with the Nova Scotia College of Social Workers or applicable College.
  • Minimum of two years of related clinical experience, preferably in pediatric settings and autism services.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Strong analytical and problem-solving skills, with the ability to navigate complex systems and prioritize tasks.
  • Ability to work collaboratively as part of an interprofessional team, promoting a culture of respect and inclusivity.
