Job Title: Client Care Supervisor RN/RPN

Job Summary:

We are seeking a highly skilled and compassionate Client Care Supervisor RN/RPN to join our team at CHATS. As a Client Care Supervisor, you will be responsible for providing client care needs assessment, case management, supervision, and clinical field support for in-home and community health care providers.

Key Responsibilities:

  • Conduct home visits and provide direct services to clients, including needs assessments, ongoing case management, and appropriate interventions in complex care situations.
  • Supervise and mentor Personal Support Workers, providing guidance and support to ensure high-quality care is delivered to clients.
  • Collaborate with the interdisciplinary team to develop and implement care plans that meet the unique needs of each client.
  • Provide education and training to clients and their caregivers on various topics, including health promotion, disease prevention, and management of chronic conditions.
  • Develop and maintain relationships with community agencies and referral sources to ensure seamless care coordination.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in client care.

Requirements:

  • Registered Nurse (RN) or Registered Practical Nurse (RPN) with current certificate of registration with the College of Nurses of Ontario.
  • Degree in Nursing or Gerontology preferred.
  • Three to five years' experience in client care, preferably in a community health setting.
  • Excellent assessment, communication, and problem-solving skills.
  • Demonstrated teaching, mentoring, and coaching skills.
  • Supervisory experience preferred.
  • Current CPR/first aid certification.
  • Proficiency in MS Word, Excel, Outlook, and database applications (GoldCare).
  • A valid Ontario driver's license and a vehicle are required for this position.
  • Ability to work outside regular business hours as required.
  • Fluency in both official languages (English and French) and other languages, an asset.
