About the Role

We are seeking a highly skilled and compassionate Client Care Supervisor RN/RPN to join our team at CHATS. As a key member of our interdisciplinary team, you will be responsible for providing exceptional client care services to seniors and their caregivers in the community.

Key Responsibilities
  • Conduct home visits and assessments to identify client needs and develop personalized care plans.
  • Supervise and mentor Personal Support Workers to ensure high-quality care delivery.
  • Provide direct services to clients, including needs assessments, ongoing case management, and interventions in complex care situations.
  • Foster key relationships with other agencies and referral sources to ensure seamless care coordination.
  • Collaborate with the interdisciplinary team to develop and implement care plans that meet client needs and preferences.
Requirements
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) with current certificate of registration with the College of Nurses of Ontario.
  • Degree in Nursing or Gerontology preferred.
  • Three to five years' experience in senior care or provision of client services in the community.
  • Excellent assessment, communication, and problem-solving skills.
  • Demonstrated teaching, mentoring, and coaching skills.
  • Supervisory experience preferred.
  • Excellent risk, crisis, and conflict management skills.
  • Current CPR/first aid certification.
  • Proficiency in MS Word, Excel, Outlook, and database applications (GoldCare).
  • A valid Ontario driver's license and a vehicle are required for this position.
  • Ability to work outside regular business hours as required.
  • Fluency in both official languages (English & French) and other languages, an asset.
