The Edmonton Zone Continuing Care division offers services across five primary program areas: Home Living, Supportive Living, Facility Living, Transition Services, and Palliative Care. We are seeking a dedicated Speech Pathologist to become a vital part of our Facility & Supportive Living team, providing essential care and support to adults and seniors in congregate living environments.

Role Overview:
Supportive Living serves as a non-institutional alternative for individuals who can no longer reside in their own homes, emphasizing health promotion, well-being, independence, and the maintenance of functional abilities. In this role, you will collaborate within a multidisciplinary team that includes various healthcare professionals from nursing and allied health fields. This team works together to conduct home or site visits, delivering client-centered services to individuals with diverse physical, cognitive, and mental health challenges, including those with complex health needs.

Key Responsibilities:
Utilizing strong clinical and interpersonal skills, you will implement speech therapy services in alignment with regulatory and AHS practice standards, employing best practices and a client-centered approach. You will engage clients, their families or caregivers, physicians, and the broader healthcare team, as well as other internal or external agencies as necessary. Speech therapy services may include a combination of direct therapeutic interventions, relevant education, and connections to essential resources.

Through the development and/or contribution to the Care Plan, in collaboration with clients and other healthcare providers, you will ensure that services are tailored and adjusted based on the client's response to interventions.

Professional Practice:
As a Speech Language Pathologist, you will provide clinical professional practice both independently and as part of a collaborative team, which may involve external partners. Your responsibilities will encompass prevention, health promotion, identification, assessment, intervention, and case management. You will play a key role in planning, coordinating, and delivering rehabilitation services in collaboration with patients/clients, families, healthcare professionals, and other stakeholders.

Leadership and Development:
Your leadership and interpersonal skills will be instrumental in supervising students and providing clinical guidance to other staff as appropriate. You will leverage your clinical expertise to drive ongoing quality improvement, service integration, and program development.

Qualifications:
- Master's degree in Speech-Language Pathology.
- Active or eligible registration and practice permit with the Alberta College of Speech Language Pathologists and Audiologists (ACSLPA).

Additional Requirements:
- Basic Cardiac Life Support - Healthcare Provider (BCLS-HCP).
- Experience in program development, implementation of educational initiatives, and change management.
- Ability to work independently in various client settings, including congregate living, single-family homes, and multi-unit dwellings.
- Capability to lift and carry medical bags weighing approximately 25-30 lbs.
- Proficiency in computer skills, including Microsoft Office applications.

Preferred Qualifications:
- Membership in Speech-Audiology Canada.
- Training and experience in dysphagia management.
- Familiarity with McMaster Case Management training and the AHS Continuing Care Case Management Competency Framework.
- Understanding of adult learning principles.
- Experience with Meditech and Connect Care.
