Job Title: Patient Care Assistant - Behaviour Support Ontario

Mackenzie Health is seeking a highly skilled Patient Care Assistant to join our Behaviour Support Ontario team. As a Patient Care Assistant, you will provide expert care and support to residents of long-term care homes with behavioural challenges.

About the Role:

The Patient Care Assistant will work closely with our Mobile Support Team Registered Nurses and Registered Practical Nurses to enhance the quality of life for residents with responsive behaviors in their own homes. You will provide crisis-intervention services, transitional management between healthcare providers, and ongoing training for residents, families, and clinicians.

Key Responsibilities:
  • Provide personal care and assistance with activities of daily living for residents with behavioural challenges.
  • Work collaboratively with the Mobile Support Team to provide crisis-intervention services and transitional management.
  • Develop and implement individualized care plans to meet the unique needs of each resident.
  • Provide education and training to residents, families, and clinicians on behaviour support and care.
  • Participate in quality improvement processes to enhance the delivery of care.
Requirements:
  • Certification as a Personal Support Worker as per the Long Term Care Homes Act and Regulations.
  • Current Certificate of Basic Cardiac Life Support (BCLS) or willingness to obtain within three months of employment.
  • Valid driver's license and access to a reliable vehicle at all times.
Preferred Qualifications:
  • One year of experience working with geriatric clients in long-term care homes.
  • Ability to speak another language.
  • Experience with quality improvement processes.
