Position Overview

Are you dedicated to enhancing the quality of health care delivery? Personal Support Workers (PSWs) play a crucial role in the health care system. In this position, you will empower, assist, and foster independence for patients in home and community care environments by supporting their daily living activities under the guidance of our Personal Support Manager.

We are seeking a Community Resource Facilitator (CRF) to help us provide patient and family-centered care to individuals in the community. Are you the right fit for this role?

Key Responsibilities

  • Develop programs based on the identification of community needs and individual client interests
  • Ensure program designs are based on best practices and successful outcomes from similar programs
  • Maintain flexibility in programs to adapt to evolving community needs
  • Provide mentorship to personal support staff and assist with personal care for patients as needed
  • Collaborate with care coordinators to identify additional residents who may benefit from our programs
  • Support staff and patients in the safe use of key lock boxes
  • Ensure program documentation is maintained and updated regularly
  • Collect and record data related to program indicators for monthly reporting
  • Maintain Personal Emergency Response Systems (PERS) units

Qualifications

  • Valid certification as a Personal Support Worker in Ontario
  • Experience in geriatrics, enhanced through clinical practice as a PSW
  • Minimum of three years of experience in health care or community-based settings
  • Familiarity with community resources
  • Strong organizational and coordination skills
  • Proficient in facilitating group sessions and teaching
  • Excellent verbal and written communication skills
  • Proven ability to mentor and work collaboratively with others
