About the Role

We are seeking a highly skilled and experienced Nurse Practitioner to join our team at Homewood Health Centre. As a key member of our interdisciplinary team, you will play a vital role in delivering advanced practice nursing to patients and families with complex mental health needs.

Key Responsibilities
  • Health Assessments and Diagnosis: Utilize your comprehensive theoretical knowledge base and advanced clinical competence to conduct thorough health assessments and diagnose medical and cognitive problems.
  • Collaborative Care: Work closely with consulting physicians to facilitate early assessment and intervention for patients requiring consults.
  • Patient Management: Support the management of patients in collaboration with the interdisciplinary team, including comprehensive nursing/medical history, physical and psychosocial assessment of the patient's status.
  • Care Pathways: Utilize established care paths and order sets to guide care practices for the patient population from admission through to discharge.
  • Treatment Planning: Support the health team through the completion of comprehensive initial patient assessments and established individualized treatment and care plans for complex patients in collaboration with physicians and psychiatrists.
  • Community Coordination: Assist in/coordinates care with patients, families, and communities.
  • Therapeutic Relationships: Create and maintain a climate for and a commitment to healing within a therapeutic relationship.
  • Documentation: Document and communicate the patient's beliefs, values, and wishes, honoring them as much as possible given respect for others and legal, ethical, and professional boundaries.
  • Cultural Sensitivity: Demonstrate sensitivity to culture, situation, and family dynamics.
  • Patient Participation: Maximize the patient's participation and control in their own treatment, providing information and support for decision making.
  • Goal-Oriented Care: Facilitate change directed by the patient, mapped out by mutually developed goals.
  • Risk Assessment: Assess problems rapidly, noting and documenting significant changes. The Nurse Practitioner in collaboration with the nurse assesses the potential for risk accurately and demonstrates leadership and skill in extreme or life-threatening emergencies.
  • Communication: Document according to professional standards, the principles of clear communication, and Homewood policy and procedure.
Requirements
  • Experience: 3+ years of nursing experience as a NP, with mental health and addiction expertise preferred.
  • Registration: Current registration in the Extended Class (Nurse Practitioner) with the College of Nurses of Ontario is required.
  • Professional Associations: Active membership in related professional associations required, e.g. NPAO, RNAO.
  • Certifications: Mental Health Nursing certificate preferred.
  • Education: Masters of Nursing required.
  • Skills: Innovative, creative, and professional role model. Possess excellent assessment and clinical skills. Ability to prioritize/organize multiple demands. Ability to work autonomously. Working knowledge of Mental Health Act and community mental health resources.
  • CPR/Basic Rescuer: CPR/Basic rescuer certification required.
  • Specialized Knowledge: Highly skilled in assessment and treatment of psychiatric and substance use disorders, including PTSD, delirium, dementia, depression, suicide prevention, and the management of aggressive behavior.
  • Measurement-Based Care: Awareness and acknowledgement of the value of using measurement-based care when supporting patients/clients to help guide clinical decision making; experience an asset.
  • Emergency Mental Health: Current knowledge and experience specific to emergency Mental Health and crisis intervention.
