About the Role

We are seeking an experienced Advanced Practice Nurse to join our team at Homewood Health Centre. As a key member of our interdisciplinary team, you will be responsible for providing high-quality, patient-centered care to individuals with complex mental health needs.

Key Responsibilities
  • Conduct comprehensive health assessments and develop individualized care plans in collaboration with consulting physicians and other healthcare professionals.
  • Provide expert nursing care, including health counseling and emotional support, to patients and their families.
  • Collaborate with the interdisciplinary team to coordinate care and ensure seamless transitions between services.
  • Utilize established care paths and order sets to guide care practices and promote best practices.
  • Support the management of patients with complex needs, including those with psychiatric and substance use disorders.
  • Document patient information accurately and in a timely manner, adhering to professional standards and Homewood Health policies.
Requirements
  • 3+ years of experience as a Nurse Practitioner, with a focus on mental health and addiction expertise.
  • Current registration in the Extended Class (Nurse Practitioner) with the College of Nurses of Ontario.
  • Active membership in related professional associations, such as the NPAO and RNAO.
  • Mental Health Nursing certificate preferred.
  • Masters of Nursing required.
  • Excellent assessment and clinical skills, with the ability to prioritize and organize multiple demands.
  • Ability to work autonomously and as part of a collaborative team.
  • Working knowledge of the Mental Health Act and community mental health resources.
  • CPR/Basic Rescuer certification.
