As a Nurse Practitioner, you will play a vital role within a multidisciplinary team focused on providing advanced nursing care to patients and families facing complex challenges at Homewood Health Centre. Key Responsibilities:

  • Employ a comprehensive theoretical knowledge base alongside advanced clinical skills to assess and manage patients, including conducting health evaluations, diagnosing medical and cognitive issues, and formulating and executing care plans.
  • Collaborate with consulting physicians to ensure timely assessment and intervention for patients requiring specialist consultations.
  • Assist in managing patient care in conjunction with the interdisciplinary team, which includes thorough nursing and medical histories, as well as physical and psychosocial evaluations of patients' conditions.
  • Utilize established care pathways and order sets to inform care practices from patient admission through to discharge.
  • Support the healthcare team by conducting comprehensive initial assessments and developing individualized treatment plans for complex cases in partnership with physicians and psychiatrists.
  • Coordinate care efforts with patients, families, and communities.
  • Foster a healing environment through the establishment of therapeutic relationships.
  • Document and communicate patients' beliefs, values, and preferences, ensuring respect for their wishes while adhering to legal, ethical, and professional standards.
  • Demonstrate cultural sensitivity and awareness of family dynamics.
  • Encourage patient involvement and autonomy in their treatment decisions, providing necessary information and support.
  • Facilitate patient-directed changes through collaboratively developed goals.
  • Quickly assess and document significant changes in patient conditions, demonstrating leadership and expertise in critical situations.
  • Maintain documentation in accordance with professional standards and organizational policies.

Qualifications:

  • A minimum of 3 years of nursing experience as a Nurse Practitioner, with a preference for expertise in mental health and addiction.
  • Current registration in the Extended Class (Nurse Practitioner) with the College of Nurses of Ontario is mandatory.
  • Active membership in relevant professional associations is required.
  • A Mental Health Nursing certificate is preferred.
  • A Master's degree in Nursing is essential.
  • Exhibit innovative, creative, and professional role modeling.
  • Possess exceptional assessment and clinical skills.
  • Ability to prioritize and manage multiple responsibilities effectively.
  • Capable of working independently.
  • Familiarity with the Mental Health Act and community mental health resources.
  • CPR/Basic Rescuer certification is required.
  • Highly skilled in the assessment and treatment of psychiatric and substance use disorders, including PTSD, delirium, dementia, depression, suicide prevention, and managing aggressive behaviors.
  • Recognize the importance of measurement-based care in guiding clinical decision-making; relevant experience is an asset.
  • Current knowledge and experience in emergency mental health and crisis intervention.
