Key Responsibilities:
  • Employ a comprehensive theoretical knowledge base and advanced clinical skills to provide care for patients, including conducting health assessments, diagnosing medical and cognitive issues, and developing and implementing care plans.
  • Collaborate with consulting physicians to ensure timely assessment and intervention for patients requiring consultations.
  • Support patient management in partnership with the interdisciplinary team, including thorough nursing and medical histories, as well as physical and psychosocial assessments.
  • Utilize established care pathways and order sets to direct care practices from admission to discharge.
  • Assist in coordinating care with patients, families, and communities, while maintaining a commitment to a healing environment within therapeutic relationships.
  • Document and communicate patients' beliefs, values, and wishes, ensuring respect for individual preferences while adhering to legal, ethical, and professional standards.
  • Demonstrate cultural sensitivity and awareness of family dynamics.
  • Encourage patient involvement and autonomy in their treatment decisions, providing necessary information and support.
  • Facilitate patient-directed changes through collaboratively developed goals.
  • Quickly assess problems, documenting significant changes and demonstrating leadership in emergency situations.
  • Maintain documentation according to professional standards and organizational policies.
Qualifications:
  • A minimum of 3 years of nursing experience as a Nurse Practitioner, with a preference for expertise in mental health and addiction.
  • Current registration in the Extended Class (Nurse Practitioner) with the College of Nurses of Ontario is mandatory.
  • Active membership in relevant professional associations is required.
  • Mental Health Nursing certification is preferred.
  • A Master's degree in Nursing is essential.
  • Exhibit innovative thinking and serve as a professional role model.
  • Possess strong assessment and clinical skills.
  • Ability to prioritize and manage multiple demands effectively.
  • Capability to work independently.
  • Familiarity with the Mental Health Act and community mental health resources.
  • Current CPR/Basic Rescuer certification.
  • Highly skilled in assessing and treating psychiatric and substance use disorders, including PTSD, delirium, dementia, depression, and suicide prevention.
  • Understanding of measurement-based care to guide clinical decision-making is an asset.
  • Current knowledge and experience in emergency mental health and crisis intervention.
