Role Summary:

The Palliative Care Clinical Coach plays a crucial role in collaborating with community organizations, adhering to the strategic direction set by Ontario Health and the Ontario Palliative Care Network. This position focuses on driving improvements in access to and quality of palliative care services, in accordance with established frameworks.

Key Responsibilities:

  • Deliver coaching and mentorship in palliative care to enhance the competencies of staff and clinicians within community organizations.
  • Engage with community leaders to facilitate service integration and manage change initiatives, employing quality improvement strategies.
  • Provide expert consultation on complex clinical cases that exceed the capabilities of current providers.
  • Support Indigenous communities in their palliative care initiatives, ensuring collaboration with regional partners.
  • Co-develop implementation plans for regional frameworks with Ontario Health, including necessary metrics and reporting.
  • Participate in regional planning meetings, contributing to the overall palliative care implementation strategy.
  • Offer insights on coaching experiences and assist in reviewing regional progress reports.
  • Encourage collaboration across regions to enhance system integration.
  • Build and maintain relationships with key stakeholders to foster trust and respect.
  • Assist in the planning and execution of palliative care models in Ontario Health Teams.
  • Ensure community initiatives align with the guidance provided by the Ontario Palliative Care Network.
  • Assess the quality of palliative care services in community organizations critically.
  • Lead initiatives to engage and build community among participating organizations.
  • Enhance the skills of frontline providers in delivering primary-level palliative care.
  • Provide expert consultation to service providers through various communication methods.
  • Support Indigenous organizations in their palliative care efforts, as requested.
  • Guide the development of quality improvement plans to address identified gaps.
  • Occasionally provide direct patient care as needed.

Qualifications:

  • A post-secondary degree in a registered health care discipline (e.g., RN, RSW, NP) with significant experience in palliative care.
  • A minimum of five years of practical experience in palliative care is essential.
  • Familiarity with relevant legislation and reporting standards related to palliative care practice.
  • Preferred experience in supervising clinical staff and leading quality improvement projects.
  • Strong clinical coaching abilities and consultation skills.
  • Experience in research or program evaluation, with a focus on evidence-based practices.
  • Ability to work collaboratively across various healthcare settings and roles.
  • Proficiency in educational technologies and strong communication skills.
  • A commitment to person-centered care and advancing equity and inclusion in healthcare.

Work Environment:

This position requires travel across the designated region, providing both in-person and virtual support to healthcare providers.
