About the Role:

SE Health is seeking a dynamic and passionate Registered Nurse (RN) to join our team as a Palliative Care Specialist. As a key member of our care team, you will provide holistic nursing care to meet the physical, safety, emotional, and spiritual needs of clients and their families in their homes and communities.

Key Responsibilities:

  • Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client's unique needs
  • Provide point-of-care leadership as the primary provider for client populations, working in collaboration with other care team members and support functions
  • Support clients through care transitions by collaborating and connecting them with community support services in their local community
  • Participate in Quality Improvement processes to ensure excellence in care
  • Work flexible hours, including days, evenings, nights, and every other weekend

Requirements:

  • Canadian Nurses Association (CNA) specialty certification an asset
  • Ability to be flexible and adaptable with excellent organizational skills
  • Strong computer skills in MS Word, MS Excel, and other software
  • Willingness to complete Tracheostomy and Ventilation care training
