Are you a PSW who graduated in Ontario after April 1, 2023? You may be eligible for the Personal Support Worker Return of Service (PSW ROS) through Health Force Ontario, which offers a $10,000 incentive for a 12-month full-time commitment to an approved employer.
Role Summary
Our Health Care Technicians specializing in Palliative and/or Acute Medically Complex care have the unique opportunity to apply their skills daily with clients facing complex health challenges. Under the supervision of a Registered Nurse, the Care Technician will deliver direct, hands-on support to clients in their homes, collaborating closely with our nursing and Home Care programs. The Care Technician will maintain direct communication with the supervising nurse through a virtual electronic monitoring system, ensuring real-time updates on client care.
This position qualifies for a $3.00 per hour wage enhancement from the Ontario Government for all hours spent providing direct client care.
Comprehensive training will be provided to enhance skill sets and meet specific care requirements.
Key Responsibilities
- Under the guidance of the supervising nurse, the Health Care Technician observes and records clinical information in the patient's electronic medical record according to VON standards.
- Provides personal care to assist with daily living activities and medication management under the direction of the supervising nurse.
- Maintains direct contact with the supervising nurse through a real-time electronic monitoring system.
- Collaborates with the supervising nurse to develop and adjust the patient's care plan based on observed outcomes.
- Identifies additional resources needed in the home and coordinates with the scheduling office as necessary.
- Engages with the patient's family or caregiver to ensure a positive care experience.
Relationships
- Regularly communicates with Registered Nurses.
- Interacts with clients and their family members.
- Coordinates with administrative support staff.
Qualifications and Experience
- Certification as a Personal Support Worker (PSW).
- Developmental Support Worker (DSW) certification is an asset.
- Preferred experience of one to two years as a PSW in Palliative/Pediatric or acute care settings.
- Current CPR and First Aid certification.
- Knowledge of Palliative Care fundamentals (completion of Level I course or commitment to complete within the first year of employment).
- Completion of AHPCE course is advantageous.
Skills Required
- Ability to learn and demonstrate proficiency in additional clinical skills necessary for supporting daily living activities.
- Adaptability to various social and economic environments.
- Familiarity with community resources.
- Strong written and verbal communication abilities.
- Basic observation and problem-solving skills.
- Willingness to engage in ongoing skills training and apply learned skills effectively.
- Ability to work collaboratively in a multidisciplinary team while also being able to work independently.
Additional Requirements
- Access to a personal vehicle for work-related travel and possession of a valid driver's license and proof of insurance.
- Willingness to travel to surrounding communities.
- Current criminal background check, including a vulnerable sector screening.
- Ability to wear Personal Protective Equipment (PPE).
Work Environment and Physical Demands: Attention to detail; ability to lift and carry using proper techniques; perform physical activities such as bending, pushing, pulling, gripping, reaching, kneeling, and walking.
