Are you a PSW who graduated in Ontario after April 1, 2023? You may be eligible for the Personal Support Worker Return of Service (PSW ROS) program through Health Force Ontario, which offers a $10,000 incentive for a 12-month full-time commitment to an approved employer.
Role Summary
Our Health Care Technicians, specializing in Palliative and/or Acute Medically Complex care, have the unique opportunity to apply their skills daily with clients facing serious health challenges. Under the supervision of a Registered Nurse, the Care Technician will deliver direct, hands-on support to clients in their homes, working in conjunction with our nursing and Home Care services. The Care Technician will maintain direct communication with the supervising nurse through a virtual monitoring system, ensuring real-time updates.
This position qualifies for a $3.00 per hour wage enhancement from the Ontario Government for all hours spent providing direct client care.
Comprehensive training will be provided to enhance skill sets and meet specific requirements.
Key Responsibilities
- Under the guidance of the supervising nurse, the Health Care Technician will observe and record clinical information in the patient's electronic medical record according to VON standards.
- Provide personal care to assist with daily living activities and medication management under the direction of the supervising nurse.
- Maintain direct contact with the supervising nurse through a real-time electronic monitoring system.
- Collaborate with the supervising nurse to develop and adjust a patient's care plan based on observed outcomes.
- Identify additional resources needed in the home and coordinate with the scheduling office as necessary.
- Engage with the patient's family to ensure a positive care experience under the supervision of the nurse.
Relationships
- Regularly communicate and collaborate with Registered Nurses.
- Interact with clients and their family members.
- Coordinate with administrative support staff.
Qualifications and Experience
- Certification as a Personal Support Worker (PSW).
- Developmental Support Worker (DSW) certification is a plus.
- Preferred experience of one to two years as a PSW in Palliative/Pediatric or acute care settings.
- Current CPR and First Aid certification.
- Knowledge of Palliative Care fundamentals (Level I) is required, with a commitment to complete the next available course within the first year of employment.
- Completion of AHPCE course is an asset.
Skills Required
- Ability to learn and demonstrate proficiency in additional clinical skills necessary for supporting daily living activities.
- Adaptability to work in diverse social and economic environments.
- Familiarity with community resources.
- Strong written and verbal communication abilities.
- Basic observation and problem-solving skills.
- Willingness to engage in ongoing skills training and apply learned competencies.
- Ability to work effectively within a multidisciplinary team and independently.
Additional Requirements
- Access to a personal vehicle for work-related travel and possession of a valid driver's license and proof of insurance.
- Willingness to travel to surrounding communities.
- Current criminal background check, including a vulnerable sector screening.
- Ability to wear Personal Protective Equipment (PPE).
Work Environment and Physical Demands: Attention to detail; ability to lift and carry using proper techniques; physical activities including bending, pushing, pulling, gripping, reaching, kneeling, and walking.
