Are you a PSW who completed your training in Ontario after April 1, 2023? You may be eligible for the Personal Support Worker Return of Service (PSW ROS) program through Health Force Ontario, which offers a $10,000 incentive for a 12-month full-time commitment to an approved employer.

Job Responsibilities

Our Health Care Technicians, specializing in Palliative and/or Acute Medically Complex care, have exceptional opportunities to utilize their skills daily with clients requiring specialized support. Under the supervision of a Registered Nurse, the Care Technician will deliver direct, hands-on assistance to clients in their homes, working in conjunction with our nursing and Home Care programs. The Care Technician will maintain direct communication with the supervising nurse through a virtual electronic monitoring system, ensuring real-time updates.

This role qualifies for a $3.00 per hour wage enhancement from the Ontario Government for all hours spent directly caring for clients.

Comprehensive training will be provided to enhance skill sets and meet job requirements.

Key Duties

  1. Under the guidance of the supervising nurse, the HCT will observe and record clinical data in the patient's electronic medical record according to VON standards.
  2. Provide personal care to assist with daily living activities and medication management under the direction of the supervising nurse.
  3. Maintain direct contact with the supervising nurse through a real-time electronic monitoring system.
  4. Collaborate with the supervising nurse to develop and adjust a patient's care plan based on observed outcomes.
  5. Identify additional resources needed in the home and coordinate with the scheduling office as necessary.
  6. Engage with the patient's family to ensure a positive care experience under the supervision of the nurse.

Collaboration and Communication

  1. Regularly interact with Registered Nurses.
  2. Engage with clients and their family members.
  3. Communicate directly with administrative support staff.

Qualifications and Experience

  1. Certification as a Personal Support Worker (PSW).
  2. Developmental Support Worker (DSW) certification is an asset.
  3. Preferred experience of one to two years as a PSW in Palliative/Pediatric or acute care settings.
  4. Current CPR and First Aid certification.
  5. Knowledge of Palliative Care fundamentals (completion of Level I course or commitment to take it within the first year of employment).
  6. AHPCE course completion is advantageous.

  4. Strong written and verbal communication abilities.
  5. Basic observation and problem-solving skills.
  6. Experience and aptitude for ongoing skills training and implementation.
  7. Ability to work effectively in a multidisciplinary team and independently.

Additional Requirements

  1. Access to a personal vehicle for work-related travel and possession of a valid driver's license and proof of insurance.
  2. Willingness to travel to surrounding communities.
  3. Current criminal background check, including a vulnerable sector screening.
  4. Ability to wear Personal Protective Equipment (PPE).
