Key Responsibilities- Assess patients' physical and psychological needs to identify problems and develop comprehensive care plans.
- Collaborate with interprofessional teams to provide high-quality patient care and make adjustments to care plans as needed.

- Maintain accurate and detailed patient records and documents care provided.
- Participate in quality improvement activities and research to enhance patient care and outcomes.
- Graduation from an approved school of Nursing and current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Ability to assess patients and apply assessments to determine appropriate patient care interventions.
- Ability to develop and implement comprehensive plans of care.
- Knowledge of chronic management models and ability to teach and promote chronic disease self-management.
