We are seeking a highly skilled and compassionate Registered Nurse to join our team at Fraser Health. As a key member of our inter-professional team, you will provide direct patient care to clients of our Urgent Primary Care Centre, providing timely and longitudinal primary care to vulnerable clients.
Key Responsibilities- Provide initial client consultation, assessment, coaching, and teaching to ensure clients receive ease of access to timely, person-centered, continuous, and culturally safe quality health services.
- Collaborate with acute, primary, and community care healthcare providers to facilitate the flow of information and ensure seamless transitions across the healthcare continuum.
- Establish a dignifying, purposeful, and trusting relationship with clients, promoting self-determination and independence, and empowering clients and natural supports to engage and connect within their communities.
- Assess clients and collect information from various sources, making nursing diagnoses and determining whether conditions can be improved or resolved through nursing interventions or collaboration with other clinical staff.
- Process physician or nurse practitioner orders, administer medications, carry out treatments, and monitor and document effects.
- Screen referrals, provide individualized client assessments, inter-professional care planning, and interventions when appropriate, and refer clients to other services as needed.
- Develop comprehensive shared client care plans in collaboration with the inter-professional team, primary care provider, client, and/or family, and facilitate transitions to primary/community care providers and community agencies.
- Provide direct planned and unplanned client care, identify other care services required, and provide comprehensive explanations of care to clients and families.
- Review client care plans, determine timing and referral to other services, and initiate, monitor, and evaluate the appropriateness and effectiveness of short and long-term care plans.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives.
- One year recent related clinical experience in acute care, urgent/emergency care, or a related clinical area, including assessing and treating children, youth, and adults in an acute or community/outpatient care setting.
- Valid BC Driver's license and access to a personal vehicle for business-related purposes, as required.
