POSITION: LICENSED PRACTICAL NURSE (LPN)

DEPARTMENT: NURSING

SUPERVISOR: Director of Care or Designate

SUPERVISORY RESPONSIBILITIES: Health Care Aides

EDUCATION:

Completion of an accredited Licensed Practical Nursing program is mandatory.

EXPERIENCE:

Recent relevant experience in the appropriate clinical setting may be required or preferred.

PHYSICAL DEMANDS AND WORKING CONDITIONS:

Good physical and mental health is essential. The role requires moderate to heavy physical exertion. Strong communication skills are vital.

CERTIFICATION/LICENSES:

Must maintain and provide proof of registration with the College of Licensed Practical Nurses of Manitoba (CLPNM). Current Family & Friends registration at the Targeted Responder Level is necessary.

LANGUAGE:

Proficient command of the English language (both spoken and written) is required.

KNOWLEDGE:

  • Standards of Practice for Licensed Practical Nurses set by the College of Licensed Practical Nurses of Manitoba.
  • Code of Ethics for Licensed Practical Nurses as outlined by the College of Licensed Practical Nurses of Manitoba.
  • Scope of practice as defined in the Licensed Practical Nurses Act.
  • Facility and Regional policies, procedures, protocols, and guidelines.
  • Manitoba Health Standards for Personal Care Homes.
  • Nursing care principles related to the clinical area.
  • Personal Health Information Act (PHIA), Protection of Persons in Care Act, Mental Health Act, Workplace Hazardous Material Information System (WHMIS), and other relevant legislation.

  • Proficiency in MDS and Care Organizer programs.

MAIN FUNCTION:

Under the general supervision of the Director of Care, the Licensed Practical Nurse (LPN) is tasked with delivering professional nursing services to assigned residents. This includes independent nursing tasks, administering prescribed treatments, collaborating with the health care team, and participating in the coordination of resident care and discharge. The LPN is committed to upholding the Resident Bill of Rights and the organization's Mission, Vision, and Values.

KEY RESPONSIBILITIES:

Resident Care/Clinical Practice:

The LPN applies clinical knowledge and judgment to assist residents in achieving their health care objectives. The LPN practices independently within their defined scope of practice, providing holistic, compassionate, and culturally sensitive care that respects the individuality of residents and their support systems. Responsibilities include:

  1. Assessment: Conducting initial assessments with residents and families to gather comprehensive health data.
  2. Planning: Collaborating with residents and families to prioritize care needs and develop individualized care plans.
  3. Implementation: Organizing work for efficient care delivery and performing prescribed interventions safely.
  4. Evaluation: Assessing the achievement of health goals with residents and families, modifying care plans as necessary.

Leadership:

The LPN is responsible for delegating tasks appropriately and ensuring continuity of care among team members.

Communication/Documentation:

Effective communication with residents, families, and team members is essential for delivering safe care. Accurate documentation of observations and care provided is required.

Professional Responsibilities:

The LPN provides care in accordance with the Licensed Practical Nurses Act and advocates for residents' rights.

Quality Improvement:

Participation in quality improvement initiatives and maintaining clinical expertise through ongoing education is expected.

Other Responsibilities:

May be required to assume the role of Nurse-in-Charge under supervision when necessary.
