Job Summary

The Physician Navigator plays a vital role in supporting the vision and mission of Prairie Mountain Health by reducing administrative burdens on physicians and providing practice-specific support. This ambassador will interact with medical providers and allied health workers to improve navigation of available resources for patients and practices.

Key Responsibilities

  1. Develop and maintain physician orientation processes to enhance knowledge of regional and provincial services.
  2. Conduct regular facility tours to gather feedback from new physicians on additional information needed during orientation.
  3. Provide timely support through phone calls, in-person requests, and online services.
  4. Curate and maintain a directory of services in PMH in collaboration with clinics, hospitals, managers, and physician groups.
  5. Work with Diagnostic Services to understand available tests, their locations, and ordering procedures.
  6. Ensure seamless computer access for new physicians.
  7. Offer a comprehensive overview of PMH services tailored to specific communities.
  8. Arrange dedicated orientation time for new physicians with Department Chiefs, Site CMOs, or Clinic physicians.
  9. Collaborate with communities receiving new physicians to curate local resources, including family support information.
  10. Regularly check-in with new physicians to the region.
  11. Tour facilities and clinics to gather feedback from established physicians on patient navigation issues.
  12. Perform exit interviews and report to Executive Management on notable issues and patterns.
  13. Define available procedures in PMH.
  14. Outline referral processes for PMH and Winnipeg.
  15. Identify clinical issues that can be addressed virtually.
  16. Determine services requiring physician referrals versus those accessible without.
  17. Document community resources and access methods.
  18. Communicate new or updated services to physicians.
  19. Perform other duties as assigned.
