POSITION SUMMARY:
St. Joseph's Home Care is committed to delivering exceptional and empathetic support to individuals in their residences and/or alternative environments through visiting or shift care services. Registered Practical Nurses play a vital role in enhancing the quality of life for their clients by offering a comprehensive range of care with predictable outcomes, adhering to SJHC's policies and procedures as well as the standards set by the College of Nurses of Ontario. Through their dedicated efforts, Registered Practical Nurses foster the healing and autonomy of their clients while promoting the ongoing enhancement of community health.RESPONSIBILITIES: Operates autonomously in the community, delivering high-quality nursing care within their scope of practice for clients and families with established care plans and predictable outcomes. Effectively employs the nursing process to ensure superior nursing care for clients and/or caregivers across all client scenarios, integrating current knowledge and research into practice. Adheres to the nursing care plan, including discharge strategies, within the RPN's scope of practice. Collaborates with clients, the LHIN, physicians, and other multidisciplinary team members to continuously assess and adjust the care plan. Primarily provides nursing services within the Hamilton Visiting Program, while also extending care to clients in the Integrated Client Care Program as required. Maintains professional competence and engages in ongoing professional development opportunities, acquiring advanced or specialized nursing skills to address client care needs as necessary. Conducts self-evaluations in line with the College of Nurses of Ontario Standards of Practice and the SJHC appraisal framework. Advocates for clients and caregivers by clarifying information, supporting decision-making, and facilitating effective communication. QUALIFICATIONS: Current RPN registration with the College of Nurses of Ontario, free from restrictions or pending hearings, AND Practical Nursing Diploma from an accredited college program, including IV medication administration as part of the curriculum OR Practical Nursing Diploma from an accredited college program along with an IV medication administration certificate from an accredited institution if this content was not part of the basic diploma program OR Practical Nursing Certificate from an accredited college program, complemented by a medication administration certificate and an IV medication administration certificate from an accredited institution. Must possess a wound care certificate from an accredited institution. Current registration with the College of Nurses of Ontario. Basic Cardiac Life Support (BCLS) certification. Ability to operate within the full scope of practice as defined by the College of Nurses of Ontario. Experience in conducting thorough nursing assessments (e.g., Mini-mental status, depression, pain assessments). Proficient in using software for client documentation, word processing, and communication. Strong commitment to customer service. Demonstrated dedication to professional growth and continuing education. A solid record of work performance and attendance is essential. Proven commitment to the values of Dignity, Respect, Service, Justice, Responsibility, and Inquiry. Exhibits competencies in Service and Mission Focus, Achievement, Teamwork, Learning and Innovation, Communication, Conflict Resolution, and Resilience. Original and current (within six months) Police Check with Vulnerable Sector Screening. Valid Driver's License and proof of $1M automobile liability insurance. Capable of driving locally and on highways. Must be COVID-19 vaccinated.
