Job Summary

We are seeking a highly skilled and experienced Site Access Coordinator to join our team at Fraser Health. As a key member of our patient flow team, you will play a critical role in ensuring the efficient and timely placement of patients across our acute care sites.

Key Responsibilities
  • Coordinate patient placement activities, assessing current and projected capacity, prioritizing and expediting critical issues requiring immediate action.
  • Lead daily bed meetings with Patient Care Coordinators and site leadership to ensure effective bed management and patient flow.
  • Consult with the Patient Transfer Network (PTN) and community resources to identify appropriate care for patients, developing and maintaining an understanding of available services and resources.
  • Coordinate acute site transfer planning, arranging patient/resident/client transfers and identifying potential discharges and associated issues.
  • Provide clinical guidance and support to nursing staff, facilitating positive team relationships and promoting quality patient care.
  • Develop and recommend changes to patient access and flow policies and procedures, working collaboratively with the interdisciplinary team to optimize timely bed access and patient placement.
  • Analyze statistics and trends to identify opportunities for improvement, working with clerical support staff to maintain accurate bed availability data.
Requirements
  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Five years' recent, related clinical experience in an acute care, multidisciplinary facility, including nursing leadership experience and a preference for experience in critical care areas or access and planning.
  • Ability to apply continuous quality improvement tools and techniques in problem solving, supervise in a multidisciplinary setting, and prepare and present educational material.
  • Clinical competence, critical thinking, and logical reasoning skills, as well as the ability to communicate effectively and work independently and as a team member.
