Job Title: Care Transitions Coordinator- Patient Flow

At North York General Hospital, we are committed to excellence in patient care. We are seeking a highly skilled Care Transitions Coordinator- Patient Flow to join our team.

Job Summary:

The Care Transitions Coordinator- Patient Flow will be responsible for coordinating and facilitating patient flow and transitions from admission to discharge. This includes working collaboratively with physicians, allied health professionals, and community partners to ensure timely and appropriate placement of patients.

Key Responsibilities:
  • Coordinate patient admissions, discharges, transfers, and transitions using NYGH's flow and access tools, standards, and protocols.
  • Facilitate the appropriate placement of patients through daily patient placement meetings and effective communication with hospital teams.
  • Assess patient transition needs and challenges, and identify and mitigate barriers to appropriate patient discharge and care transitions.
  • Support discharge planning and workflow optimization for Resource Matching and Referrals, iPlan Platform, and Alternate Level of Care patients.
  • Capture and track data related to patient flow and transitions operations, and develop KPIs to evaluate performance.
  • Verify, correct, and submit data and compliance reports pertaining to NYGH's patient flow activities, occupancy, and ALC.
Requirements:
  • Undergraduate degree in a healthcare discipline required.
  • Current member and in good standing with a professional college required.
  • Minimum 3 years' recent clinical experience in an acute care hospital setting required.
  • Project Management (PMP Certification) and Lean Six Sigma certification preferred.
  • Ability to work independently and effectively to assess patients' placement needs.
  • Proven leadership skills.
  • Data quality management and performance evaluation.
  • Strong facilitation and conflict resolution skills.
  • Ability to assess risk and effectively utilize resources to support patient acuity.
  • Excellent Microsoft Office applications skills.
  • Bilingualism in French/English an asset.
