Resident Care Coordinator

We are seeking a highly skilled and dedicated Resident Care Coordinator to join our team at Sunrise Senior Living. As a key member of our leadership team, you will be responsible for providing overall leadership and management of the long term care neighborhood, ensuring the delivery of high-quality care and services to our residents.
Key Responsibilities- Champion the Resident Centered model and ensure its implementation throughout the neighborhood.
- Develop and lead the overall operation of the long term care neighborhood, ensuring compliance with federal, provincial, and local laws and regulations.
- Facilitate the development and implementation of Individualized Care Plans (ICPs) and Daily Assignment Sheets.
- Manage the assessment process to determine levels of care and ensure appropriate staffing levels.
- Partner with the activities and volunteer coordinator to ensure a variety of activities are available to residents.
- Communicate regularly with families and physicians as needed.
- Ensure compliance with Sunrise standards and policies, including quality assurance and regulatory compliance.
- Assist in the presentation and value of Sunrise's products and services to residents, families, and referral sources.
- Manage the department budget and ensure timely submission of financial reports.
- Develop and implement plans of action to correct identified deficiencies in compliance with Sunrise expectations.
- One year experience in assisted living long term care or hospital industries or experience/exposure to the senior population.
- College degree and management experience may be required per provincial requirements.
- One year supervisory and management experience, including hiring staff, coaching, performance management, daily operations supervision, discipline, and counseling.
- Ability to handle multiple priorities and delegate assignments to team members.
- Competent in organizational and time management skills, with good judgment, problem-solving, and decision-making skills.
- Proficient in computer skills, Microsoft Office, and Sunrise applications.
