Unit Overview

The Community and Population Health Services (CPHS) division is dedicated to enhancing the integration of health and social services, bridging the gap between hospital care and community support. This program is pivotal in fostering innovative collaborations with peers, community organizations, and academic institutions, while providing expert guidance in both pre- and post-hospital transitions.

CPHS oversees critical strategies including Population Health, Bundled Care, and Alternate Level of Care. The division extends system-wide assistance through various roles such as Site Administrative Managers, Managers of Transitions, Transitional Care & Discharge Specialists, ALC Navigators, and the Centralized Care and Transition Team. This includes initiatives like Hospital to Home (H2H) and the Integrated Comprehensive Care (ICC) Program, alongside the Medical Psychiatry Consultation Liaison Team and Substance Use Services, in partnership with Home & Community Care Support Services.

Role Overview

The Nurse Practitioner (NP) within the Community & Population Health Program is a registered professional with the College of Nurses of Ontario in the Extended Class - Adult, functioning within the RN-EC scope of practice.

As an integral member of the Centralized Care and Transition Team (CCaTT), the NP collaborates with a multidisciplinary team, the Emergency Department, and other healthcare professionals, utilizing a patient and family-centered approach. Furthermore, the CCaTT NP engages in the Virtual Nursing Care Program, providing support for patients with COPD and heart failure, adhering to telepractice standards set by the College of Nurses of Ontario and organizational policies. CCaTT NPs deliver direct patient care, which includes assessing and treating individuals with complex health needs, formulating integrated care plans for seamless transitions from hospital to community, and conducting post-discharge follow-ups.

At Hamilton Health Sciences, NPs play a vital role in mentoring healthcare professionals, participating in ongoing education, leading initiatives, and enhancing the effectiveness of interdisciplinary care teams. The CCaTT NP works closely with program leadership to promote operational excellence and sustainability.

Work Schedule

Monday to Friday, with occasional weekend and holiday shifts.

Required Qualifications

  • Valid Certificate of Registration in the College of Nurses of Ontario Extended Class - Adult
  • Master's Degree in Nursing
  • At least 5 years of recent clinical experience with older adults facing complex health challenges
  • In-depth understanding of chronic disease management frameworks
  • Familiarity with community resources for effective discharge planning
  • Ability to critically evaluate literature and apply best practices
  • Knowledge of quality improvement and leadership principles
  • Strong communication and presentation abilities
  • Experience in implementing innovative initiatives through change management
  • Excellent problem-solving, organizational, and prioritization skills
  • Critical thinking and interpersonal skills
  • Adaptability to evolving clinical situations
  • Proficiency in information technology

Proof of full COVID-19 vaccination is mandatory for employment. Proficiency in both English and French is considered an asset.
