Job Description:

The Advanced Practice Nurse (APN) will work in collaboration with physicians, other members of the interdisciplinary team, and clients/families to provide evidence-informed and integrated care to registered outpatients of the Schizophrenia Outpatient Clinic and Transitional Outpatient Program for Schizophrenia Service. The APN will be responsible for providing autonomous healthcare services, grounded in professional, ethical, and legal standards, using advanced nursing knowledge, skills, experience, clinical reasoning, problem-solving, and critical thinking.

Key Responsibilities:

  • Provide autonomous healthcare services to registered outpatients of the Schizophrenia Outpatient Clinic and Transitional Outpatient Program for Schizophrenia Service.
  • Work in collaboration with physicians, other members of the interdisciplinary team, and clients/families to provide evidence-informed and integrated care.
  • Develop and implement individualized care plans that address the unique needs and goals of each client.
  • Provide education and support to clients and their families to promote health and wellness.
  • Collaborate with community resources and services to ensure seamless care transitions.

Qualifications:

  • Current registration with the College of Nurses of Ontario in the RN-Extended Class with specialty certification as APN.
  • Master's Degree in Nursing required.
  • Proof of successful completion of controlled substances education required.
  • Faculty appointment or eligibility for appointment with a School of Nursing in an academic institution.
  • CNA certification in Psychiatric/Mental Health Nursing (CPMHN(c)) preferred.
  • Demonstrated experience with psychiatric assessment and providing pharmacologic and non-pharmacologic interventions.
  • At least three years recent demonstrated experience as a Nurse Practitioner with experience in community-based settings servicing individuals diagnosed with schizophrenia and related psychotic disorders.

Working Conditions:

The APN will work in a fast-paced, dynamic environment, providing care to clients with complex mental health needs. The APN will be required to work collaboratively with a multidisciplinary team, including physicians, nurses, social workers, and other healthcare professionals. The APN will also be required to work independently, making autonomous decisions and taking responsibility for the care of clients.
