Physiotherapist - Home and Community Care Specialist

We are seeking a highly skilled Physiotherapist to join our team at Home and Community Care Support Services. As a Physiotherapist, you will play a vital role in providing high-quality patient care and support to individuals in their homes and communities.
Key Responsibilities- Assess and determine patient care needs and eligibility for home and community care services.
- Provide access and referrals to community services and resources.
- Engage with patients, caregivers, and other healthcare professionals to ensure seamless care coordination.
- Develop and implement individualized treatment plans to promote patient independence and well-being.
- Conduct home visits and provide physiotherapy services to patients in their homes and communities.
- Collaborate with multidisciplinary teams to ensure comprehensive patient care.
- Maintain accurate and up-to-date patient records and reports.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Master's degree in Physiotherapy or equivalent.
- Current registration with the College of Physiotherapists of Ontario.
- Minimum 2 years of experience in a physiotherapy role, preferably in a community-based setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong problem-solving and critical thinking skills.
- Fluency in English and French is required.
- Vulnerable Sector Check completed within the last six months.
