Job Title: Physiotherapist
Job Summary: We are seeking an experienced Physiotherapist to provide patient-centered care and support the well-being of our patients in a community-based setting.
Key Responsibilities:
- Assess and determine patient care needs and eligibility for community services.
- Provide access and referrals to community services, engaging with patients, caregivers, and other healthcare practitioners.
- Plan, organize, and direct diverse programs of treatment, counseling, screening, and education.
- Establish priorities and treatment goals with respect to identified problems.
- Maintain accurate clinical records in accordance with Ontario Health atHome Champlain policies and procedures.
- Function as a member of a multi-disciplinary team and maintain liaison with other healthcare professionals.
- Participate in committees as approved.
- Participate in professional development activities and maintain knowledge and ability in the realm of physiotherapy.
- Maintain supplies and recommend equipment for department use within budget limitations.
- Interpret the role and objectives of the Ontario Health atHome Champlain Physiotherapy Program in the community.
Requirements:
- Degree or diploma in Physiotherapy and current certificate of registration with the College of Physiotherapists of Ontario.
- Related professional experience, normally attained over a period of two years in a Physiotherapy role.
- Related experience in a similar community-based setting is an asset.
- Current CPR-C certificate.
- Fluency in English and French is an asset.
- Vulnerable Sector Check completed within the last six months.
