Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our Home and Community Care Support Services team. As a Nurse Practitioner, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes and communities.
Key Responsibilities- Provide comprehensive assessments and develop individualized care plans to meet the unique needs of patients.
- Collaborate with patients, families, and healthcare providers to ensure seamless care coordination and communication.
- Prescribe medications and order diagnostic tests as needed to ensure optimal patient outcomes.
- Develop and implement strategies to promote patient safety, prevent hospitalizations, and improve quality of life.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care.
- Current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse in the Extended Class (RN(EC) designation).
- Minimum 2 years of experience as a Nurse Practitioner in a community or home care setting.
- Advanced assessment and clinical skills, with a focus on palliative care, complex chronic disease management, and psychogeriatric care.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a focus on collaboration and coordination of care.
