Physiotherapist - Home and Community Care Support Services

We are seeking a skilled Physiotherapist to join our team at Home and Community Care Support Services. As a Physiotherapist, you will play a vital role in providing high-quality care to our patients in their homes and communities.
Job Summary:The successful candidate will be responsible for assessing and determining patient care needs, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners. You will work collaboratively with our team to provide connected, accessible, patient-centered care.
Key Responsibilities:- Assess and determine patient care needs and eligibility
- Provide access and referrals to community services
- Engage with patients, caregivers, and other healthcare practitioners
- Plan, organize, and direct diverse programs of treatment, counseling, screening, and education for patients
- Establish priorities and treatment goals with respect to identified problems and develop methods of achieving these goals
- Maintain accurate clinical records in accordance with Home and Community Care Support Services policies and procedures
- Function as a member of a multi-disciplinary team and maintain liaison with other healthcare professionals
- Act as an education and resource person for other healthcare professionals
- Participate in committees as approved
- Partake in professional development activities and maintain knowledge and ability in the realm of physiotherapy
- Maintain supplies and recommend equipment for department use within budget limitations
- Interpret the role and objectives of the Home and Community Care Support Services Physiotherapy Program in the community
- Degree or diploma in Physiotherapy and current certificate of registration with the College of Physiotherapists of Ontario
- Related professional experience which would normally be attained over a period of two years in a Physiotherapy role
- Related experience in a similar community-based setting is an asset
- Current CPR-C certificate
- Fluency in English and French is required
- Vulnerable Sector Check completed within the last six months
