Job Summary

We are seeking a skilled Physiotherapist to join our team at Home and Community Care Support Services. As a Physiotherapist, you will play a vital role in providing high-quality patient care and support in the Eastern Counties region.

Key Responsibilities
  • Assess and determine patient care needs and eligibility, providing access and referrals to community services.
  • Engage with patients, caregivers, and other healthcare practitioners to deliver connected, accessible, patient-centered care.
  • Plan, organize, and direct diverse programs of treatment, counseling, screening, and education for patients in various settings.
  • Establish priorities and treatment goals, developing methods to achieve these goals and ensuring clinical records are maintained in accordance with policies and procedures.
  • Function as a member of a multi-disciplinary team, maintaining liaison with other healthcare professionals and acting as an education and resource person.
  • Participate in committees as approved and engage in professional development activities to maintain knowledge and ability in physiotherapy.
Requirements
  • Degree or diploma in Physiotherapy and current certificate of registration with the College of Physiotherapists of Ontario.
  • Related professional experience, typically acquired over a period of two years in a Physiotherapy role.
  • Related experience in a similar community-based setting is an asset.
  • Current CPR-C certificate.
  • Fluency in English and French is an asset.
  • Vulnerable Sector Check completed within the last six months.
