Job Title: Physiotherapist
Job Summary: As a Physiotherapist with Home and Community Care Support Services, you will play a vital role in delivering patient-centered care and supporting the well-being of our patients. Your expertise will be essential in assessing and determining patient care needs, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners.
Key Responsibilities:
- Assess and determine patient care needs and eligibility
- Provide access and referrals to community services
- Engage with patients, caregivers, and other healthcare practitioners
- Plan, organize, and direct diverse programs of treatment, counseling, screening, and education
- Establish priorities and treatment goals with respect to identified problems
- Maintain accurate clinical records in accordance with Home and Community Care Support Services policies and procedures
- Function as a member of a multidisciplinary team and maintain liaison with other healthcare professionals
- Participate in committees as approved
- Participate in professional development activities and maintain knowledge and ability in the realm of physiotherapy
- Maintain supplies and recommend equipment for department use within budget limitations
- Interpret the role and objectives of the Home and Community Care Support Services Physiotherapy Program in the community
Requirements:
- Degree or diploma in Physiotherapy and current certificate of registration with the College of Physiotherapists of Ontario
- Related professional experience, normally attained over a period of two years in a Physiotherapy role
- Related experience in a similar community-based setting is an asset
- Current CPR-C certificate
- Fluency in English and French is an asset
- Vulnerable Sector Check completed within the last six months
