Traverse Independence is seeking a highly skilled and dedicated Service Coordinator to join our team. As a Service Coordinator, you will play a vital role in supporting individuals with acquired brain injuries (ABI) to live independently in the community.
Key Responsibilities- Leadership and Supervision: Directly supervise front-line staff, providing coaching, team building, supervision, performance reviews, and performance management to ensure high-quality services.
- Program Evaluation and Oversight: Conduct data collection and evaluation of individual client services and the overall program, providing leadership to ensure equitable access to high-quality services.
- Client and Staff Support: Notify the manager of client and staff issues or concerns that cannot be resolved immediately or are considered high-priority items.
- Accreditation and Compliance: Support all procedures and processes required to maintain an exemplary standing with Accreditation Canada.
- Technology and Integration: Value and capably use a fully integrated SharePoint and O365 environment to support client services.
- Health and Safety: Ensure the health and safety of both staff and clients.
- Client Goal-Based Programs: Ensure clients in goal-based programs have all required assessments, documentation, and service plans completed.
- Integration and Collaboration: Ensure integration with other sectors and workers involved in client care teams to support wrap-around services for clients.
- Professional Culture: Maintain a professional, collaborative, and integrative perspective and culture in the program.
- Ethics and Risk Management: Work within an ethical framework, risk management system, and quality improvement philosophy.
- Confidentiality and Privacy: Have a clear and dedicated understanding of privacy and confidentiality.
- Education: Community College Diploma in Social Services, Healthcare, or other related discipline.
- Experience: A minimum of 2 years' experience in social services or healthcare with at least 1 year supervising or leading a team.
- Specialized Knowledge: Experience in the mental health and/or addiction field, demonstrated experience in behavior management programs, and comprehensive knowledge and experience working in a SharePoint environment, including Microsoft 365 Teams.
- Skills: Problem-solving and conflict resolution skills, excellent communication skills (written and oral), experience working with persons with disabilities and/or acquired brain injuries, and understanding of community resources, with an emphasis on mental health and addictions.
- Additional Requirements: Valid Class G driver's license and reliable vehicle, with travel required.
