Key Responsibilities:

* Provide case management services to clients, including assessment, planning, and coordination of services
* Develop and implement individualized service plans to meet the unique needs of clients
* Collaborate with other agencies and organizations to ensure comprehensive support for clients
* Maintain accurate records and reports of client interactions and service delivery
* Participate in team meetings and contribute to the development of program policies and procedures

Requirements:

* Bachelor's degree in Social Work or a related field
* 1-2 years of experience in social services or a related field
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Bilingualism (English and French)
