As a Community Care Coordinator, your responsibilities will include evaluating and identifying patient care requirements and eligibility, facilitating access to community resources, and collaborating with patients, their families, and other healthcare professionals.

Whether you are based in an office, working within the community, or at a healthcare facility, you will be instrumental in delivering cohesive, accessible, and patient-focused care. You will be part of a supportive team comprising over 9,000 regulated healthcare and allied professionals.

Key Responsibilities
  1. Collaborate with patients and their families to assess care needs, establish eligibility for services, and create personalized care and service plans.
  2. Connect patients with appropriate service providers.
  3. Oversee and manage the implementation of care plans.
  4. Build supportive relationships with patients and their families.
  5. Balance patient preferences and needs with available resources, ensuring respect for their values and choices.
Essential Qualifications
  1. Active membership in the relevant regulatory body.
  2. A minimum of 2 years of recent experience in community health or a related sector.
  3. Understanding of the healthcare delivery system and community resources.
  4. Strong interpersonal, communication, assessment, problem-solving, and decision-making abilities.
  5. Effective time management, prioritization, and organizational skills, with the capacity to work both independently and collaboratively in a dynamic multidisciplinary setting.
  6. Proven ability to accurately complete necessary documentation, reports, and forms.
  7. A valid driver's license and access to a dependable vehicle.
  8. Proficiency in a Windows operating environment.
  9. Compliance with our mandatory COVID-19 vaccination policy, requiring proof of vaccination status prior to employment commencement.
Preferred Attributes
  1. Experience working with diverse patient populations, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatric, and pediatric groups.
  2. Background in case management or relevant community experience.
  3. Ability to communicate in French or another second language.
