Key Responsibilities
  1. Collaborate with patients and their families to assess care needs and develop tailored service plans.
  2. Connect patients with appropriate service providers.
  3. Oversee the implementation and monitoring of care plans.
  4. Establish supportive relationships with patients and their families.
  5. Balance patient preferences with available resources, ensuring respect for their values.
Essential Qualifications
  1. Active membership with the relevant regulatory body.
  2. Minimum of 2 years of recent experience in community health or a related area.
  3. Familiarity with the healthcare delivery system and community resources.
  4. Strong interpersonal, communication, assessment, problem-solving, and decision-making abilities.
  5. Effective time management and organizational skills, with the capacity to work independently and collaboratively in a dynamic environment.
  6. Proven ability to accurately complete necessary documentation and reports.
  7. A valid driver's license and access to a reliable vehicle.
  8. Proficiency in a Windows operating environment.
  9. Compliance with the mandatory COVID-19 vaccination policy.
Preferred Qualifications
  1. Experience with diverse patient populations, including multicultural, homeless, palliative, mental health, and geriatric groups.
  2. Background in case management or relevant community experience.
  3. Ability to communicate in French or another second language.
