Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our team at Fraser Health. As a key member of our Overdose Response Team, you will play a critical role in providing clinical and consultative case management services to priority clients and their natural supports.

Key Responsibilities
  • Conduct comprehensive social work and psychosocial assessments to identify client needs and develop effective care plans.
  • Provide therapeutic clinical counseling and crisis intervention services to clients and their natural supports.
  • Work collaboratively with the interdisciplinary team to coordinate services, manage cases, and facilitate meetings.
  • Ensure follow-up and support client engagement along the continuum of care.
  • Provide harm reduction education, counseling, and support, including episodic overdose prevention services and drug checking.
  • Establish and maintain effective relationships with community social service agencies to identify gaps in services and improve client care.
  • Participate in clinical consultation and education within the team regarding psychosocial factors affecting client care and engagement.
  • Maintain accurate and up-to-date client records, including referrals, assessments, and progress notes.
Requirements
  • Bachelor's degree in Social Work from an approved school of Social Work.
  • Current full registration with the BC College of Social Workers.
  • Two years recent, related experience in a complex healthcare environment, including one year experience working with clients who have been systematically marginalized with multiple health issues.
  • Valid B.C. driver's license and access to a personal vehicle for business-related purposes.
  • Current First Aid and CPR Level C.
Preferred Qualifications
  • Demonstrated ability to work with priority and culturally diverse populations related to substance use and the toxic drug-poisoning crisis.
  • Demonstrated orientation to practice that deeply listens to and honours the knowledge of the people we serve, including radical allyship with people who use drugs, harm reduction, cultural humility and safety, sex-positive, trauma and resiliency-informed practice.
  • Knowledge of legislation and government policies and procedures that relate to substance use practices and how they may affect client care, resource access or practice guidelines.
