Job Summary

Fraser Health is seeking a highly skilled and compassionate Substance Use Outreach Coordinator to join our team. As a key member of our interdisciplinary team, you will provide critical support services to individuals and their families struggling with substance use issues.

Key Responsibilities
  • Provide support services to clients and their families, including accessing opioid agonist therapy and related support services.
  • Monitor clients' health conditions and environments, documenting behavioral, physical, and cognitive changes as needed.
  • Arrange and escort clients to medical appointments and errands, providing emotional, moral, and physical support as required.
  • Create linkages to other community resources and programs, advocating on behalf of clients as necessary.
  • Assist clients in accessing benefits and assistance, providing brief assessments and planning on issues related to substance use, opioid agonist therapy, overdose prevention, and harm reduction.
  • Administer medication to clients under the direction of a Registered Nurse, providing medication reminders as needed.
  • Liaise with community resources and healthcare providers, promoting development of life-skills in clients and providing basic health-promoting information.
  • Perform general clerical duties, including maintaining client files and utilizing computer software applications.
  • Attend meetings and participate in patient care planning and coordination.
Requirements
  • Completion of a Substance Use Certificate, plus one year's recent related experience working with clients with substance use issues.
  • Valid BC driver's license and vehicle.
  • Current CPR certificate.
  • Demonstrated ability to work with marginalized and culturally diverse populations, including those with substance use issues and mental illness.
  • Excellent communication and interpersonal skills, with the ability to establish rapport with clients and families.
  • Working knowledge of harm reduction principles, crisis prevention/intervention, and conflict resolution/de-escalation skills.
