Job Summary

We are seeking a skilled Community Rehabilitation Specialist to join our team at St. Paul Abilities Network (Society). As a key member of our rehabilitation team, you will work closely with clients to provide support and guidance in achieving their rehabilitation goals.

Key Responsibilities
  • Client Support: Provide individualized support to clients with physical disabilities, focusing on rehabilitation and reintegration into the community.
  • Assessment and Planning: Conduct thorough assessments of clients' needs and develop personalized rehabilitation plans to address their goals and objectives.
  • Community Linkages: Establish and maintain relationships with community resources and services to ensure clients have access to necessary support and care.
  • Documentation and Reporting: Maintain accurate and detailed records of client progress and outcomes, and provide regular reports to team members and management.
Requirements
  • Education: College, CEGEP, or other non-university certificate or diploma from a program of 3 months to less than 1 year.
  • Experience: 7 months to less than 1 year of experience in a related field.
  • Certifications: Valid certifications in areas such as first aid, CPR, and crisis intervention.
  • Skills: Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
