We are seeking a skilled Community Rehabilitation Specialist to join our team at St. Paul Abilities Network (Society). As a key member of our rehabilitation team, you will work closely with clients to provide support and guidance in achieving their rehabilitation goals.
Key Responsibilities- Client Support: Provide individualized support to clients with physical disabilities, mental health conditions, or other challenges, to help them achieve their rehabilitation goals.
- Assessment and Planning: Conduct assessments to identify client needs and develop personalized rehabilitation plans, in collaboration with healthcare professionals and other stakeholders.
- Community Outreach: Establish and maintain relationships with community resources, services, and organizations to ensure clients have access to necessary support and services.
- Documentation and Reporting: Maintain accurate and comprehensive records of client progress, including assessments, plans, and outcomes.
- Education: College, CEGEP, or other non-university certificate or diploma from a program of 3 months to less than 1 year.
- Experience: 7 months to less than 1 year of experience in a related field.
- Certifications: Valid certifications in areas such as first aid, CPR, or other relevant areas.
- Skills: Excellent communication and interpersonal skills, ability to work independently and as part of a team, and strong organizational and time management skills.
