Job Summary

We are seeking a skilled Community Rehabilitation Specialist to join our team at St. Paul Abilities Network (Society). As a key member of our team, you will play a vital role in supporting individuals with disabilities to achieve their goals and live independently.

Key Responsibilities
  • Education and Training
    • Provide education and training to individuals with disabilities to help them develop essential life skills.
    • Collaborate with community resources to provide access to educational programs and services.
  • Case Management
    • Assess the needs of individuals with disabilities and develop personalized plans to support their goals.
    • Coordinate with healthcare professionals, family members, and other stakeholders to ensure comprehensive support.
  • Community Outreach
    • Build relationships with community organizations and resources to provide access to services and support.
    • Develop and implement community-based programs to promote social inclusion and empowerment.
  • Documentation and Reporting
    • Maintain accurate and up-to-date records of client interactions, progress, and outcomes.
    • Prepare and submit reports to stakeholders, including government agencies and funding bodies.
Requirements
  • Education
    • College, CEGEP, or other non-university certificate or diploma from a program of 3 months to less than 1 year.
  • Experience
    • 7 months to less than 1 year of experience in a related field.

    • Ability to work independently and as part of a team.
    • Excellent communication and interpersonal skills.
    • Strong problem-solving and critical thinking skills.
    • Ability to work under pressure and meet deadlines.
