Community Rehabilitation Specialist

We are seeking a skilled Community Rehabilitation Specialist to join our team at St. Paul Abilities Network (Society). As a key member of our team, you will play a vital role in supporting individuals with disabilities to achieve their goals and live independently.
Key Responsibilities- Education and Training
- Provide education and training to individuals with disabilities to help them develop essential life skills.
- Collaborate with community resources to provide access to educational programs and services.
- Case Management
- Assess the needs of individuals with disabilities and develop personalized plans to support their goals.
- Coordinate with healthcare professionals, family members, and other stakeholders to ensure comprehensive support.
- Community Outreach
- Build relationships with community organizations and resources to provide access to services and support.
- Develop and implement community-based programs to promote social inclusion and empowerment.
- Documentation and Reporting
- Maintain accurate and up-to-date records of client interactions, progress, and outcomes.
- Prepare and submit reports to stakeholders, including government agencies and funding bodies.
- Education
- College, CEGEP, or other non-university certificate or diploma from a program of 3 months to less than 1 year.
- Experience
- 7 months to less than 1 year of experience in a related field.

- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
