Job Summary

We are seeking a highly skilled Licensed Practical Nurse to join our team at Providence Health Care. As a key member of our hemodialysis unit, you will provide exceptional patient care and work collaboratively with our interdisciplinary team to deliver high-quality services.

Key Responsibilities
  • Assess, plan, implement, evaluate, and document patient care during pre-dialysis, dialysis, and post-dialysis periods.
  • Perform patient admission duties, including assessing condition, individual patient needs, and planning nursing care in collaboration with the RN and other members of the nephrology team.
  • Collaborate with the RN and other members of the nephrology team to provide nursing duties related to hemodialysis, such as weighing patients, cannulation of established fistula or graft, and continuous heparin regulation.
  • Monitor patients, recognize changes, and communicate with RN any problems, changes, or unusual signs or symptoms of the patient while in the dialysis setting.
  • Document observations, treatment, and other patient care activities during the pre, intra, and post-dialysis periods.
  • Work collaboratively with RN regarding patient assignment to assess changes in patients, both physical and emotional, and determine the appropriate level of care required.
Requirements
  • Graduation from a recognized program for Practical Nurses with the full range of current licensed practical nursing competencies.
  • Successful completion of a recognized Dialysis Program for Licensed Practical Nurses.
  • One (1) year recent related experience working as a Licensed Practical Nurse in acute care.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives.
  • Current basic cardiac life support certification.
