Key Responsibilities- Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
- Provide direct patient care, including hemodialysis treatments, and monitor patient progress.
- Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication.
- Participate in quality improvement initiatives and contribute to the development of nursing policies and procedures.
- Provide education and support to patients and families, promoting health literacy and self-management skills.
- Lead and participate in interdisciplinary teams to achieve patient-centered care goals.
- Current practicing registration as a Registered Nurse with the BC College of Nurses and Midwives.
- Completion of a post-graduate program in Nephrology nursing or one year of recent experience in hemodialysis.
- Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
- Ability to prioritize tasks, manage multiple responsibilities, and maintain accurate records.
- Physical ability to perform the duties of the position, including lifting and transferring patients.
