Job Summary

Halton Healthcare is seeking a highly skilled and compassionate Advanced Practice Registered Nurse to join our team. As a Palliative Care Specialist, you will play a critical role in providing high-quality patient care and support to individuals and families in our community.

Key Responsibilities
  • Provide comprehensive health assessments and develop individualized plans of care for patients with complex medical needs.
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between healthcare settings.
  • Develop and implement strategies to improve patient outcomes and quality of life.
  • Provide education and support to patients, families, and healthcare providers on palliative care and end-of-life issues.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures related to palliative care.
Requirements
  • Current registration in the Extended Class (NP-Adult) with the College of Nurses of Ontario.
  • Completion of a Master Degree in Nursing.
  • Minimum of 3 years nursing experience in palliative care.
  • Exceptional knowledge in palliative care and pain & symptom management.
  • Formal training and education in Palliative Care with minimum of LEAP Course completion.
  • Experience and expertise in areas of palliative and end of life care, pain and symptom management, supporting individuals with complex medical needs and working with diverse communities and agencies.
  • Eligibility for cross-appointment to an academic institution.
  • Membership in specialty related organizations NPAO, RNAO.
  • Innovative, creative and professional role model.
  • Demonstrated ability to prioritize/organize multiple demands.
  • Ability to work autonomously.
  • Proficient with Microsoft Office applications including Outlook, Word, Excel, PowerPoint and Visio.
  • Ability to meet the physical demands of the position.
  • Satisfactory employment and attendance record.
