The job entails facilitating the coordination of integrated service delivery responses to vulnerable clients and other responsibilities as delineated below:
**Major Responsibilities**:

- Pre-screens nominated risk situations based on masked data.
- Facilitates and co-chairs the SPIDER Situation Table (including implementing structured information sharing protocols).
- Supports systems coordination efforts by working with City service areas, including Toronto Public Health, Toronto Fire Services, Emergency Medical Services, Toronto Police Service, Toronto Community Housing Corporation, Municipal Licensing and Standards, Shelter, Support and Housing Adminstration and community based organizations and health care services providers, to ensure integrated and more effective responses to health and safety risks involving Torontonians experiencing vulnerability, including their homes or property and/or their neighbours.
- Assesses SPIDER situations, gathers and disseminates information, documents and supports the Situation Table decisions and recommendations.
- Facilitates the development and implementation, with key stakeholders, of appropriate strategies to improve access to services by building partnerships that promote inter-sectoral linkages.
- Provides advice, mentoring, consultation support, information and referrals to potential City divisional staff, community leaders, service providers, and to community organizations with the goal of facilitating effective integrated responses.
- Acts as a resource for City staff, politicians and other external stakeholders in identifying and acting on current and anticipated issues and providing regular updates.
- Works with and assists other divisional and City staff, and institutional and community partners by providing consultation, education materials and staff training, and facilitation on vulnerability, access and equity, and advocacy to build capacity for integrated response (including accountabilities, legalities, and policies and procedures) in coordination with City Human Resources Division and the Equity, Diversity, and Human Rights Office.
- Communicates and interprets relevant policies, programs and initiatives to stakeholders.
- Evaluates SPIDER situation outcomes and performance to ensure service responses are integrated and effective, and makes recommendations for improvement as necessary.
- Develops and maintains a shared performance measurement database.
- Monitors results and performance, writes reports, including developing protocols and policy recommendations as needed, in collaboration with Participating Divisions and partners.
- Acts as a conduit between the Situation Table and the Systemic Reform efforts
- Provides Staff support to Systemic Reform efforts including reports and initiatives.

**Key Qualifications**:

- Extensive experience in working with vulnerable communities to address social inequities, manage trauma, and develop community mental health supports, including the effective use of creative and innovative approaches.
- Experience in facilitating inter-sectoral responses, community partnership development, infrastructure and capacity building.
- Experience designing, developing and delivering a variety of training tools and outreach material to support inter-divisional capacity building.
- Experience in writing grant proposals, including the development of budgets and submitting to a variety of funding bodies.
- Degree in Social Sciences, Social Work, Human Services Management, or related field or approved equivalent combination of education and/or experience.
- Ability to establish and maintain effective working relationships with a variety of community and sector organizations, all City service areas, residents, representatives from various levels of government, elected officials and other related stakeholders.
- Knowledge of risk driven community safety models, system navigation, community services ensuring community appropriate responses and support.
- Ability to work within an anti-oppression framework, including familiarity with the sociology of large and complex urban areas with an ethno-racially, linguistically and economically diverse population.
- Ability to support the Toronto Public Service values to ensure a culture that champions equity, diversity and respectful workplaces.
- Strong conflict resolution, interpersonal and problem-solving skills and the ability to develop consensus and recommend effective, strategic solutions at the inter-divisional leve
