***:Journey Home Hospice (JHH) Toronto, operated by Saint Elizabeth Foundation, is seeking a talented professional Coordinator of Psychosocial Services. The hospice offers a specialized trauma-informed, culturally safe, and harm-reducing approach to care, serving patients and their chosen family who have experienced homelessness and other structural vulnerability. The coordinator will bring a master’s in social work complemented by experience and knowledge about life-altering diagnoses, hospice palliative care, and end-of-life care. Prior experience or interest in working with patients and their loved ones who have experienced homelessness and other structural vulnerability would be a strong asset.

Journey Home Hospice offers an engaging, interdisciplinary, and dynamic workspace where you will primarily work onsite at the hospice or at community outreach partner sites, with some virtual support of patients outside of the GTA.

**POSITION RESPONSIBILITIES**:
**Psychosocial Services**
- Receive referrals from the hospice interdisciplinary team and responds to patients within response guidelines.
- Complete psychosocial assessments, make recommendations regarding appropriate social work intervention. Provides individual and family counseling according to identified needs where appropriate.
- As part of interdisciplinary team design and implement group counseling/programs that addresses common psychosocial needs of patients and chosen family members.
- Communicate, implement, and ensure that JHH’s trauma-informed, culturally safe, and harm-reducing approach to care is maintained.
- Work with patients on advance care planning, accessing social services (OHIP, ODSP, PG&T, etc.) and funeral arrangements.
- Communicate and implement any of the support services available if appropriate for patients and their chosen family.
- Assess for psychosocial education needs and assist to provide education to staff and volunteers.
- Participate in team meetings for the purposes of case consultation and information sharing.
- Complete required initial file documentation, ongoing updates, and monthly statistical reports, prioritizes duties on caseload.
- Attend any meetings, conferences etc. for the purpose of coordinating services and/or professional development.
- Supervise and manage clinical placements for social work students.
- Enter all clinical notes and other relevant data into PointClickCare electronic health record.

**Community Outreach & Wellness Activities**
- In coordination with the Outreach Nurse, meet with potential patients to determine goals of care and suitability for admission. Enter information into RM&R as necessary if patient comes through non-medical referral source. Coordinate admission care planning with the Director of Care and provide pre-admission report and notifications to JHH interdisciplinary staff and volunteers.
- Develop and nurture professional and therapeutic relationships with community partners.
- Research, develop, and run psychosocial wellness activities at JHH and community partner sites where JHH has formal outreach agreements. For example, individual or group counselling for potential patients, chosen family members, etc.
- Develop and run grief and bereavement services for the chosen family members of JHH patients in Toronto and Windsor such as individual or group counselling, bi-annual memorial events, and other relevant activities in collaboration with Spiritual Care.
- Assist the Volunteer, Wellness, and Non-Clinical Operations Manager in developing other wellness programs for patients and/or community outreach locations that have positive psychosocial benefits that may use volunteers to implement activities

**Reporting and Evaluation**
- Develop quality evaluation and reporting metrics including a dashboard to provide monthly real-time feedback on the program directives.
- Prepare narrative evaluations of patient outcomes, program strengths and identified gaps, as well as remain abreast of leading development in research and evaluation of the Journey Home Hospice program and other similar programs.
- Regularly prepare written reports for submission to program funders, the Saint Elizabeth Foundation Board of Directors, donors, and other key collaborators.
- Personal lived experience with homelessness, structural vulnerability, or self-identification as a person of marginalized community would be considered an asset.

**Other Duties**
- Attend public relations and/or fundraising events as required.
- Attend media interviews, communications outreach, or other public relations special events as requested. Other duties as assigned.
