Service Coordinator for Rehab Services

The Service Coordinator for Rehab Services will manage staff scheduling and other administrative duties, audit unbooked shift based on-rosteritng system to ensure continuity of care. The primary function of the position is also to assist rehab support workers and clients to achieve an optimal level of independence. Under the supervision of a regulated healthcare professional, the rehabilitation support coordinator will coordinate and provide support services to clients who suffer from the effects of an acquired brain injury, mental health issues, spinal cord injuries and/or various complex needs. The coordinator will provide life skills retraining, behavioural management, cognitive remediation, community reintegration, educational and vocational support. Excellent communication and computer skills are required.

RESPONSIBILITIES:

- Work collaboratively with the Clinical Manager to ensure continuity of care.
- Ensure continuity of services in the event of staff absences/vacancies; prepare a coverage plan.
- Assist in implementing communication strategies to ensure staff are informed of all organization, program and discipline specific issues or relevant information
- Adjust/communicate/manage/maintain updated the scheduling.
- Assign unbooked shifts based on rostering system to ensure continuity of care in the provision of service deliver.
- Maintain and update contact staffing directory lists.
- Maintain/update existing profiles in the schedule/ staffing calendar.
- Process all same day or next day schedule changes and update lists.
- Inform front line staff of same day or next day changes to their schedules/new shifts
- Provide follow-up on missed shifts and escalate patterns as appropriate
- Communicate with other team memebers efficiently.
- Assist in ordering, organizing and maintaining office and therapy supplies.
- Prepare accurate documentation.
- Perform all other reasonable duties as assigned administrative duties as assigned

Education & Experience
- Relevant post-secondary education within the health or social services field.
- Minimum of five years of experience in acquired brain injury and/or mental health.
- In-depth knowledge and experience with the design, development and implementation of behavioural modification strategies and interventions.
- Demonstrate excellent interpersonal and communication skills.
- Personal computer skills including familiarity with Microsoft Office software.

**REQUIREMENTS**:

- Maintain vehicle insurance coverage of at least $2 million liability and a rider to carry clients.
- A clear driver’s abstract
- Current criminal records check
- Certified in First Aid and CPR
- Current immunization
