FAMILY SUPPORT WORKER - EASTERN ONTARIO BLIND LOW VISION AND INFANT HEARING

**Status**: Regular Part-time, 28 hrs/week

**Pay rate**: $28.219/hr to $33.317/hr

Job Summary
The Family Support Worker (FSW) will work with families of children diagnosed with permanent hearing loss and/or vision impairment. The Family Support Worker will be responsible for providing families with initial support in coping and adjusting to the diagnosis, ensuring the development of partnerships with families and service providers, facilitating access to information and resources for the families and providing case management/communication plan coordination supports as required.

Job Specific Responsibilities
- Participate in Diagnostic meetings with Audiology for Deaf or Hard of Hearing children.
- Provide grief counseling to parents of children recently diagnosed as Blind or Low Vision and/or Deaf or Hard-of-Hearing.
- Provide support and access to information regarding various communication interventions options to the family to facilitate their readiness to make a decision with respect to the Communication Development Plan for their child who is Deaf or Hard-of-Hearing.
- Assist families to access the appropriate services, once they have chosen a method of communication for their child.
- Provide on-going support to assist family with coping and family adjustment.
- Link families with community service and support agencies; initiate and facilitate referrals as necessary; assist in coordinating appointments as required.
- Provide case management for services specific to blind-low vision, as required.
- Provide Communication Development Plan coordination specific to children who are Deaf or Hard-of-Hearing, as outlined in the Communication Development Plan process.
- Organize multidisciplinary team meetings to support families’ 6 F-Words goals for their child and develop plan of care.
- Write comprehensive Communication Development Plan reports from the meetings (English or French)
- Advocate for and assist families in obtaining funding for respite
- Support families with their child’s school transition by explaining school options, making referrals and facilitating meetings with the Itinerant Teachers of the Deaf and Hard-of-Hearing
- Regular outreach phone calls for all families in Blind Low Vision and/or Infant Hearing Program
- Facilitate Parent Support Groups and playgroups for families in Blind Low Vision and/or Infant Hearing Program
- Conduct home visits with Early Childhood Vision Consultants to assess family’s needs and plan care accordingly.
- Participate in regular Ontario Collaborative Family Support Worker meetings
- Update the Specialized Preschool Program Manager and the team about families as required.
- Develop a strong partnership with CTC, CHEO Audiology and Ophthalmology, community agencies, and service providers.
- Link families with other parents of children who are blind or low vision and/or who are Deaf or Hard-of-Hearing as appropriate.
- Link Blind and/or Low Vision parents of a sighted child with community resources and information to support their child’s healthy growth and development.
- Link Deaf and/or Hard of Hearing parents of a hearing child with community resources and information to support their child’s healthy growth and development.
- Complete appropriate documentation including a record of family contacts and data tracking information for the regional Blind/Low vision program and for the regional Infant Hearing Program
- Provide education to parents and childcare teachers.
- Forward necessary documentation to the appropriate sources in a timely manner.
- Work in collaboration with the Early Childhood Vision Consultants in the Blind Low Vision Program.
- Work in collaboration with Audiologists, Auditory Verbal Therapists, and ASL/LSQ Consultants in Infant Hearing Program
- Attending both provincial and local training specific to the position, as required.
- Perform other duties, as assigned, in accordance with professional standards and guidelines.

**Qualifications**:

- Bachelor’s of Social Work
- Fluency in English and French is essential.
- Minimum three years of clinical counseling skills regarding family support and decision-making specific to children with special needs
- Experience with blind-low vision and deaf or hard-of-hearing children and their families is an asset.
- Local experience with service providers working with blind or low vision and deaf or hard of hearing children.
- Knowledge and experience in grief counselling.
- Knowledge of resources available to the blind-low vision and deaf community
- Excellent communication and interpersonal skills,
- Sensitivity to people of different cultural and socio-economic backgrounds.
- Experience working with families and infants.
- Valid driver’s license and access to a vehicle as travel is required.
