To help fulfill its mission, LOFT is hiring a **Case Manager**.
**Job Summary**
As a Case Manager you will work to coordinate a support network for clients living with mental health challenges, substance use issues, physical health issues and experiencing homelessness. The position attempts to empower the client, thereby increasing their independence, dignity and self-respect. Direct support will be provided to clients in service navigation, housing and shelter access, linkages to physical and mental health care, goal setting, crisis intervention, and advocacy.
**Positive Service Coordination (PSC)** is a program within McEwan Case Management that provides short-term intensive Case Management for adults 16 or older living with HIV or AIDS and experiencing homelessness, mental health challenges and/or substance use issues. PSC also provides services to individuals who identify as trans, non-binary and gender fluid and require case management support.
**What You Will Do**
- Develop a plan of support and recovery which includes identifying priorities and goals in collaboration with the client and other service providers, and as identified through intake assessments, including OCANs;
- Promote the personal empowerment of each individual through case planning, mentoring, accompaniment, and support in the development of life skills, including accessing services, accessing and maintaining housing, attending appointments, and managing activities of daily living;
- Work within a formal partnership with community agencies to coordinate care, reduce barriers to services, and collaboratively problem-solve to address complex challenges experienced by clients;
- Establish and coordinate a support network of social, medical, psychiatric and other community links. Develop a coordinated care plan as appropriate;
- Provide service navigation and advocacy in regards to accessing shelters; safe beds; and transitional, supportive and permanent housing appropriate to the needs of clients;
- Provide crisis intervention and assist in the development of safe plans with clients;
- Participate in the administration of the program by completing and maintaining the required records, assessments or statistics such as client files in accordance with established procedures;
- Participate in planning, organizing, promoting and conducting events for clients to address social needs and reduce isolation; maintain an awareness of community events and activities to promote the social inclusion of clients;
- Support with staff vacation and sick coverage; orient new staff, students or volunteers as required
**What You Bring**:
Must Haves
- An excellent understanding of harm reduction, and psychosocial rehabilitation principles as acquired through a relevant degree/diploma in human services
- At least three years of experience working with individuals living with HIV/AIDS who have experienced homelessness and mental health and/or substance use barriers
- At least three years of experience supporting individuals in 2SLGBTQ+ communities with demonstrated skill in working with trans, non-binary and gender fluid individuals, including knowledge of relevant community services
- An understanding of the therapeutic relationship with an ability to negotiate with clients on goals and priorities
- Strong skills in working within formal partnerships, including maintaining timely and thorough communication, collaborative problem-solving, and maintaining strong inter-agency relationships
- Excellent interpersonal, relationship-building, communication and counselling skills
- Understanding of anti-oppressive principles and trauma-informed care
- Good analytical, time management and organizational skills to maintain consistency in client support
- Good intake assessment, crisis intervention, advocacy and problem-solving skills
- Computer literacy sufficient to produce and maintain files, records or statistical information
**Work Environment**:
